Just out of curiosity, why did the board fail?  About 4-5 months ago, I changed the 12AU7 tubes to 6SN7 tubes with a convert tube socket... Do you think that has anything to do the failure.

When the power line voltage is above spec (greater than 120v) the C4S feeding the shunt regulator dissipates a lot of heat. It's within spec but we had enough problems that we switched to the larger transistor not long after the upgrade board came out. At the same time, I increased the conservatism factors I use when designing! Often we saw scorch marks on the PC board, which was the first clue.
