DC and AC Voltage on Chassis when Headphones plugged in [resolved]

First build, everything looks great.  All resistance and voltage checks are good.  When I insert a 1/4 adapter into the headphone jack (testing with crummy earbuds), I get voltage on the chassis.  It goes back to ground when removed.  I did continuity checks on every point that should be a ground, and everything

240vDC and about 6VAC.  Kind of scratching my head.  Has anyone dealt with this and or is this is how the circuit is designed.

EDIT: PENDING REPAIR... Possible Discovery of issue.

Was missing a junction on the headphone jack, I did not follow instructions properly and did not junction the bottom lugs together hence the absence of a ground when the headphones were inserted.

Fun builds never leave you with 100%  success.
