I flipped the connections at the RCA inputs which fixed the issue.

With the signal carrying wire, it looks like you have black wired to the center post of each RCA jack, and it looks like you've grounded those connections up at the pot.  If this is true, this is backwards.

Yes, that was the issue - it was wired backwards.  :(
