The D cells are wired in reverse. It's an error in the manual; pins A1/A7 and B1/B7 should be 1.5v greater than A5 and B5 respectively. (This has come up a few times in the Quickie forum, but has not made it to "sticky" status yet.) This change will bring up the plate voltages (A2-4-6 and B2-4-6).
Still, your cathode voltages (T2 and T4, as well as A5, B5) are lower than they should be. Check the orientation of the bypass cap; if reversed it may be reducing the effectiveness of the cathode resistor.
Those are the only problems I see.
