While testing terminals for voltages I accidentally touched the test lead between A9 and A8 and the A8 LED went out. Ironically this was after I fixed my problem of the other LED not coming on. The following terminal voltages all read 144 while the manual says they should be around 100: 1,7,A6,B1,B3. All the other readings look good.

I am assuming I blew out the LED but if anyone knows for sure or knows anything about the voltage readings please let me know. Thanks

Yes, you blew out the LED and it must be replaced.  (See HLMP-6000 at Mouser)

For future reference, a glowing HLMP-6000 will have 1.5V across it, there isn't any need to measure it (in fact, we took the A3/A8 voltage out of the manual to prevent this from happening).
